Jacob’s missing two permanent teeth. Now what?
I regularly receive calls from orthodontists and pedodontists regarding their early teen patients because of hypodontia, a condition in which a person is missing one or more teeth. While congenitally missing incisors only occur in approximately two percent of the population, it is a condition that can be easily addressed with early identification, proper planning, and collaboration between the patient’s primary care dentist, oral surgeon and orthodontist.
Here’s a quick look at “Jacob’s” case, a straightforward example of how to address congenitally missing teeth.

Jacob’s pedodontist identified that he was missing two lower central incisors.
After Dr. Kur conferred with Jacob’s parents, he recommended that Jacob consult with an orthodontist. Jacob began a two-year course of orthodontics at age 15, at which time I was called in to be part of the treatment team. Jacob’s orthodontist devised a plan to move existing teeth while making space for the missing lower incisors. Our intent was to place implants when Jacob was fully grown. While this was underway, natural-looking artificial teeth were “built in” to his braces and retainers. As part of his dental treatment, before placing implants, Jacob wore “Maryland” bridges—a false tooth with metal wings–for a significant time. I saw him when he was 16, and 3D images were taken to evaluate if there was enough bone in his jaw for an implant. There was. A few months ago, Jacob came to me to have his two implants placed. The procedure took 30 minutes with Jacob under light sedation. Immediately following, he went to the dentist (zero waiting period) for the restoration.
Jacob is going off to college next month with now another reason to smile.
